The Clinical Simulation Exam (CSE) is unlike any test you've taken before. There are no multiple-choice questions, no single right answers, and no way to guess your way through it. The CSE tests your clinical decision-making in real time — and that requires a completely different preparation strategy than the TMC. This week-by-week CSE exam prep timeline will help you build the clinical reasoning skills you need to pass, whether you're preparing over 4 weeks, 6 weeks, or 8 weeks.
Understanding the CSE Format First
Before you can prepare effectively, you need to understand exactly what the CSE tests. The exam consists of 22 clinical simulations — 10 are scored, 12 are unscored pretest items. Each simulation presents a patient scenario and asks you to make a series of clinical decisions, with the scenario evolving based on your choices.
The CSE uses a unique scoring system. You earn points for selecting appropriate actions and lose points for selecting harmful or contraindicated ones. 4-Week CSE Prep Timeline
Week 1: Master the Format and Core Disease States
Days 1–2: Understand the CSE Scoring System
This is non-negotiable. Many students fail the CSE not because they lack clinical knowledge, but because they don't understand how the scoring works. Study the NBRC's CSE candidate handbook. Practice identifying the difference between optimal, acceptable, and harmful responses.
Days 3–4: Obstructive and Restrictive Disease Simulations
COPD and asthma exacerbations are among the most common CSE scenarios. Practice working through a COPD exacerbation from initial assessment through treatment escalation and discharge planning. Know when to initiate NIV, when to intubate, and when to wean.
Days 5–7: Infectious and Inflammatory Disease
Pneumonia, ARDS, and sepsis-related respiratory failure are high-yield CSE topics. Practice the full clinical pathway: assessment → diagnosis → treatment → monitoring → modification. For ARDS, know the Berlin Definition and lung-protective ventilation strategy cold.
Week 2: High-Acuity Scenarios and Mechanical Ventilation
Days 8–10: Mechanical Ventilation Management
The CSE will test your ability to initiate, manage, and wean mechanical ventilation. Practice setting initial parameters, troubleshooting alarms, and making appropriate adjustments based on ABG results and patient response. Know your weaning criteria (RSBI, NIF, spontaneous breathing trial).
Days 11–12: Cardiovascular and Neuromuscular Disease
Heart failure with pulmonary edema, pulmonary embolism, and neuromuscular diseases like Guillain-Barré and myasthenia gravis are tested. For neuromuscular disease, focus on monitoring respiratory muscle strength and timing of ventilatory support.
Days 13–14: Trauma and Post-Operative Scenarios
Chest trauma (pneumothorax, hemothorax, flail chest) and post-operative respiratory complications are common CSE scenarios. Know the clinical presentation and immediate management for each.
Week 3: Neonatal/Pediatric and Specialty Topics
Days 15–17: Neonatal and Pediatric Scenarios
Neonatal scenarios are among the most challenging for students who haven't had extensive clinical experience in this area. Focus on RDS, BPD, meconium aspiration, and neonatal resuscitation. Know the differences between neonatal and adult ventilation — the CSE will test whether you can apply the right approach to the right patient.
Days 18–19: Sleep Disorders and Pulmonary Vascular Disease
OSA, obesity hypoventilation syndrome, and pulmonary hypertension are tested. Know CPAP/BiPAP titration for sleep disorders and the role of pulmonary vasodilators (inhaled nitric oxide, epoprostenol) for pulmonary hypertension.
Days 20–21: Full Simulation Practice
Work through complete CSE simulations from start to finish. Time yourself. Practice making decisions under pressure. Review every response — not just the ones you got wrong, but why the optimal responses were optimal.
Week 4: Refinement and Test Readiness
Days 22–25: Targeted Weak Area Practice
Identify the disease categories where you're losing the most points and spend focused time on those scenarios. Don't try to re-learn everything — focus on the specific decision points where you're making suboptimal choices.
Days 26–28: Timed Full-Length Practice
Complete two or three full-length CSE practice sessions under real exam conditions. The CSE is 4 hours long — build your stamina now so you're not mentally exhausted by the final scenarios on test day.
Days 29–30: Light Review and Logistics
Review your most important clinical decision frameworks. Confirm your test center location, arrival time, and required identification. Get a full night of sleep.
6-Week and 8-Week Timelines
If you have more time, use it wisely. A 6-week timeline allows you to spend an additional week on simulation practice and a week on targeted review. An 8-week timeline gives you the luxury of a slower pace through the disease categories and more time for full-length practice sessions.
The key principle regardless of timeline: practice simulations early and often. The CSE is a skill, not just a knowledge test. The more simulations you work through, the more natural clinical decision-making becomes.

- Recognizing when a patient is deteriorating and responding appropriately
